The 1973 edition, in typescript, has here been reproduced in facsimile, but with the publisher name and address on the title page removed and replaced by those of Health Research in a modern font. However, the true date of composition is surely much earlier than 1973, since Perry is recorded by Holden as having died in 1961 at the age of 90. There are no library records available of editions before 1973 under the present title, so dating is impossible, except that it must be before 1962 but after 1924, since Perry is described on the title page as “Successor to Dr. Geo. W. Carey”, and Carey died in 1924
